Post Acute Services Coordinator

Job Summary:

The Post Acute Services Coordinator is responsible for the day to day supervision of the Navigation staff and the seamless transition of patients from inpatient acute care services and MD offices to outpatient or post acute care services. The PAS Coordinator is responsible for maintaining the position of liaison between Care Coordinators, MD offices and other post acute service providers. The PAS Coordinator continuously works with the in-patient and out-patient Care Coordination and hospital team to expedite discharge of patients in a timely and safe manner.

Core Responsibilities and Essential Functions:

People:

  • Responsible for supervision of direct reports and productivity management.
  • Responsible for employee engagement, department retention, and turnover.
  • Completes mid-year and yearly performance reviews for direct reports.
  • Works with HR and Manager to provide corrective actions and resolve complex issues timely.
  • Assists direct reports as necessary to expedite discharges.

Quality and Patient Safety:

  • Ensures patients are directed to appropriate level of post acute care services.
  • Complete screening tools and qualifiers for appropriate discharge.
  • Accountable for compliance with any regulatory or accreditation standards as appropriate.
  • Monitor direct reports for compliance in safety programs.

Customer Service:

  • Maintain seamless transition for patients from inpatient acute and MD office to outpatient post acute services.
  • Maintain a positive professional liaison relationship with all members of the inpatient acute, MD offices and outpatient post acute service teams.
  • Attend and participate in discharge planning meetings to expedite timely and appropriate discharges.
  • Serves as resource to perform any service recovery as necessary.

Financial:

  • Assists Manager in maintaining the operating budget for the department.
  • Ensures staff productivity.
  • Identify cost cutting measures within the department.

Successful Growth:

  • Assists Manager regarding department strategic planning and growth.
  • Maintains positive relationships with inpatient acute, MD offices and outpatient acute services teams to continue successful departmental growth.
  • Assists Manager during implementation of new programs or services.

Complies with all Wellstar Health System policies, standards of work, and code of conduct.

Required Minimum Education:

High school diploma from an accredited school of nursing State of Georgia license. Associate's Degree from an accredited school of nursing State of Georgia license. Bachelor's Degree from an accredited school of nursing State of Georgia license.

Required Minimum License(s) and Certification(s):

All certifications are required upon hire unless otherwise stated.

  • RN - Reg Nurse (Single State) or RN-COMPACT - RN - Multi-state Compact
  • BLS - Basic Life Support or BLS-I - Basic Life Support - Instructor

Additional License(s) and Certification(s):

Required Minimum Experience:

Minimum 1 year coordinating post acute services such as Home Health, Home Medical Equipment, LTAC, Hospice, SNF, etc. Required and Previous Supervisory experience Preferred

Required Minimum Skills:

Strong leadership, communication both written and verbal, problem solving and time management/organizational skills. Must be able to work independently, maintain a positive attitude and calm disposition. Proficient computer skills. Knowledge of community resources and post acute regulations. Knowledge of eligibility and coverage requirements of post-acute services.
