Job Summary

Responsible for completing initial in-home assessments of patients enrolled into the Wellstar MCG Health Virtual Care @ Home programs. This includes developing the initial care plan, communication with the patient, family, and caregivers around the programs, goals and expectations. The Admissions RN works in coordination with the Manager, Lead Care Coordinator and Virtual Care @ Home Care Team to enroll and consent patients into the program, while coordinating in-person assessments with the patient and family.

Core Responsibilities and Essential Functions

In-Home Visits: Completes initial in-person visits for patients enrolled into Wellstar MCG Health's Virtual Care @ Home program. The in-home visits include an initial assessment of patient and family to coordinate care plan, treatment goals, and ancillary needs. Performs as needed in-home visits for patients enrolled into the program in coordination with Lead Care Coordinator and Paramedics.

Patient Identification & Enrollment: Actively identifies and enrolls patients from AUMC Inpatient, ER, or clinic setting into one of the Wellstar MCG Health Virtual Care @ Home programs. This identification includes a clinical assessment based on patient's condition and discharge goals, and coordinates expected treatment plan with Virtual Care Physician for referral review and approval.

Patient Satisfaction & Advocacy: Demonstrates the ability to educate patients and family/caregivers on Virtual Care @ Home program and goals, self-care techniques, medications, disease management, and review of treatment plan. Identifies social barriers or needs that impact patient's outcomes and satisfaction of the program which would results in readmissions or unnecessary hospital visits.

Performance Improvement: Seeks opportunities to improve the Virtual Care @ Home Admissions and Enrollment process in collaboration with the AVP, Manager, Lead, Medical Director, and Service Chief. Keeps current with industry based best practices, while seeking out innovative ways to drive patient satisfaction and improved program outcomes.

Team-Based Care Planning: Coordinates enrollments and patient consents with the Virtual Care @ Home Admitting Specialist including patient demo of equipment, capturing of patient consent, coordination with Inpatient care team, and Virtual Care @ Home Care Team. Works with Virtual Care @ Home Social Worker, Inpatient Social Worker and Inpatient Care team on timely discharge from the facility into Virtual Care @ Home to improve health system throughput goals.

Performs other duties as assigned. Complies with all Wellstar Health System policies, standards of work, and code of conduct.

Required Minimum Education

  • Associates Nursing or Diploma (Nurse) Nursing or Bachelors Nursing
  • Masters Nursing-Preferred

Required Minimum License(s) and Certification(s)

All certifications are required upon hire unless otherwise stated.

  • ACLS-P - ACLS-Provisional (180 Days) within 180 Days or ACLS-I - ACLS - Instructor or ACLS - Advanced Cardiac Life Support
  • BLS-I - Basic Life Support - Instructor or BLS - Basic Life Support
  • RN - Reg Nurse (Single State) or RN-COMPACT - RN - Multi-state Compact

Required Minimum Experience

Minimum 3 years acute care nursing for ICU or medical-surgical units Required or Minimum 5 years Home Health or Hospice Preferred
