✨ About The Role
- The Nurse Case Manager II is responsible for patient case management, longitudinal engagement, discharge planning, and outpatient patient management through the care continuum
- The role involves identifying, screening, tracking, monitoring, and coordinating care for patients with multiple co-morbidities and/or psychosocial needs
- The Nurse Case Manager will develop patients' action plans and discharge plans, provide continuity of care, and act as an advocate for patients and their families
- Responsibilities include engaging patients telephonically, providing member education, conducting transition discharge assessments, and collaborating with the care team to establish individualized transition plans
- The Nurse Case Manager will also be responsible for maintaining in-depth knowledge of company products and services, as well as customer issues and needs through ongoing training and self-directed research
âš¡ Requirements
- Experienced nurse with a minimum of 5-8 years in clinical settings, preferably with diverse clinical experience caring for acutely ill patients with multiple disease conditions
- Strong knowledge of utilization management, quality improvement, and discharge planning
- Ability to work independently, prioritize tasks, and handle multiple demands simultaneously
- Excellent problem-solving skills and ability to identify community resources
- Strong interpersonal skills, including conflict resolution and negotiation abilities
