✨ About The Role
- Responsible for patient case management, discharge planning, transition of care needs, and outpatient patient management
- Engage with patients, families, and caregivers to establish a well-coordinated action plan and assess health status
- Provide member education for self-management goals, disease management, and acute conditions
- Collaborate with the Interdisciplinary Care Team to establish individualized transition plans for patients
- Maintain in-depth knowledge of company products and services, customer issues, and health care delivery systems
âš¡ Requirements
- Requires 2+ years of managed care and/or case management experience
- Knowledge of managed care, medical terminology, referral process, claims, and ICD-10 codes is essential
- Proficiency in Microsoft applications and Excel is preferred
- Strong planning, organizing, conflict resolution, negotiation, and interpersonal skills are necessary
- Ability to work independently and meet case management goals and initiatives
