✨ About The Role
- Responsible for patient case management, longitudinal engagement, and coordination for discharge planning
- Engage with patients, families, and caregivers to establish well-coordinated action plans and assess health status
- Provide member education for self-management goals, disease management, and acute conditions
- Collaborate with the care team to make referrals to community sources and programs for patients
- Utilize motivational interviewing techniques, health education, and treatment decision support for patients
âš¡ Requirements
- Minimum of 2 years of managed care and/or case management experience, with a background in clinical work
- Strong knowledge of medical terminology, referral processes, and ICD-10 codes
- Excellent written and verbal communication skills to effectively interact with patients, care teams, and medical professionals
- Ability to work independently and efficiently manage caseloads while meeting case management goals
- Current, unrestricted LPN/LVN license specific to the state of employment
