Registered Nurse – Homecare

We are currently seeking a dedicated Registered Nurse to join our Homecare team in Essex County, NJ. This role is responsible for providing skilled nursing care to patients, supporting their health and well-being through compassionate, community-based care.

This position serves a community-specific territory within Essex County. Nurses in this role are scheduled within these communities, providing consistency in scheduling and day-to-day responsibilities.

Responsibilities

What You Will Do:

• Assess and evaluate patients with multiple disease processes and comorbidities.
• Develop, implement, and update individualized care plans.
• Document changes in patient conditions and progress toward goals.
• Identify patient and family teaching needs and implement appropriate strategies.
• Monitor the quality and appropriateness of services delivered.
• Collaborate with healthcare providers and interdisciplinary team members.
• Follow agency policies for infection control, safety, and documentation standards.

Qualifications

• RN with Bachelor's degree in Nursing preferred
• 1+ year of supervised clinical experience as a registered nurse
• Excellent verbal and written communication skills
• Strong computer skills
• Excellent organizational and interpersonal skills
• Car and valid driver's license

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $78,734.56 to USD $152,400.00. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
