Position Highlights:
- Improve the quality of health for Medicare Advantage members across the states of Kansas and Missouri by helping to monitor and manage the members alongside the primary care teams. Provide telemedicine to other states within the region as needed.
- Typical hours are Monday-Friday, 8am-5pm.

Primary Responsibilities:
- Conduct assessments including an evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam.
- Patients will be seen in a variety of venues that include home visits, visits within primary care clinics, visits within our 3 community centers, visits within skilled nursing facilities, physician practices or telemedicine visits.
- Perform evidence-based practice screenings including point of care testing (as appropriate).
- Identify diagnoses to be used in care management and active medical management in the furtherance of treatment.
- Perform transition of care visits for patients recently discharged from acute or post-acute settings.
- Formulate a list of current and past medical conditions using clinical knowledge and judgment and the findings of your assessment.
- Communicate findings in your assessment that will be used to inform member's PCP of potential gaps in care.
- Identify urgent and emergent situations and intervene appropriately.
- Educate members on topics such as disease process, medication, and compliance.

Required Qualifications:
- Current NP license in the state of Kansas and Missouri, or ability to attain
- Board Certified or in the process of obtaining certification
- 1+ years of Primary Care experience
- Driver's License and access to a reliable transportation
Preferred Qualifications:
- Fluency in EMR systems & technology
- Experience conducting Annual Wellness Visits or transition of care visits or same day care
- Ability to work in a dynamic and changing setting
Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $89,900 to $160,600 annually based on full-time employment. We comply with all minimum wage laws as applicable.
