We are seeking a dynamic and experienced leader to serve as the Integrated Community Care Team Manager. In this role, you will oversee the performance and operations of contracted care teams across multiple vendor partners. You'll collaborate closely with internal leadership and clinical directors to ensure the delivery of high-quality, integrated case management services that align with organizational goals and regulatory standards.

If you reside in Colorado, you will have the flexibility to work remotely as you take on some tough challenges.

Primary Responsibilities:

  • Provide strategic and operational oversight of vendor-managed care teams, ensuring alignment with clinical and business objectives
  • Lead day-to-day operations of the Care Management Department, including staffing, workflow optimization, and performance monitoring
  • Partner with the Director and Medical Directors to define and manage a comprehensive case management and concurrent review program
  • Ensure seamless integration of care management functions across departments and vendor teams, proactively identifying and resolving barriers to coordination
  • Monitor and evaluate program outcomes, implementing process improvements and corrective actions as needed
  • Lead regulatory readiness efforts, including audits and site visits, and develop action plans to address any identified gaps
  • Develop and maintain policies, procedures, and training programs to ensure compliance with all applicable regulations
  • Collaborate on budget planning and management, ensuring cost-effective operations within established financial parameters
  • Maintain confidentiality and uphold organizational values in all interactions and decisions
  • Travel as required to vendor sites and community locations; reliable transportation and valid driver's license required

Required Qualifications:

  • Bachelor's degree in nursing, healthcare management, related healthcare field, or the combination of education and experience in the managed care industry that would enable performance of the full scope of the position
  • 5+ years related progressive experience
  • 3+ years of management experience
  • Driver's License and access to a reliable transportation

Preferred Qualification:

  • CCM certification

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). No matter where or when you begin a career with us, you'll find a far-reaching choice of benefits and incentives. The salary for this role will range from $89,900 to $160,600 annually based on full-time employment. We comply with all minimum wage laws as applicable.
