- As a Telephone Case Manager RN at UnitedHealth Group, you will provide education, advocacy, and resources to a diverse member population.
- Your role involves making outbound calls and receiving inbound calls to assess members' current health status.
- You will identify gaps or barriers in treatment plans and provide patient education for self-management.
- The job includes making referrals to outside sources and supporting members with condition education, medication reviews, and connections to resources such as Home Health Aides or Meals on Wheels.
- You will provide a complete continuum of quality care through close communication with members via in-person or on-phone interaction.

- You should be a licensed Registered Nurse with an unrestricted license in the state of employment.
- At least 3 years of experience in a hospital, acute care, or direct care setting is required.
- Proficiency in typing and navigating a Windows-based environment is essential.
- A Bachelor of Science in Nursing (BSN) and certification as a Case Manager (CCM) would be advantageous.
- Experience in discharge planning, utilization review, concurrent review, risk management, or a telephonic role would be beneficial.
- Case management experience and a background in managed care are preferred.
- You must be adaptable, patient-focused, and able to handle a high-volume, customer service environment efficiently.
- The ability to work remotely with adherence to UnitedHealth Group’s Telecommuter Policy is necessary if residing within a 1-hour distance or 100-mile radius from the hub office.
