- Assess, plan, and implement care strategies individualized by member needs
- Identify and initiate referrals for member transition to community resources
- Manage the care plan throughout the continuum of care as a single point of contact
- Communicate with all stakeholders to ensure quality coordinated care and services are provided
- Advocate for members and families to ensure their needs and choices are fully represented and supported
⚡ Requirements
- Current RN unrestricted license in the State of TX
- 2+ years of experience working within the community health setting or in a health care role
- Familiarity with Microsoft Office, including Word, Excel, and Outlook
- Ability to travel locally to visit Medicaid members in their homes and other settings
- Experience with long term care services and support, Medicaid, or Medicare
