✨ About The Role
- Assess, plan, and implement care strategies individualized by patient and directed toward the most appropriate level of care
- Identify and initiate referrals for social service programs and manage the care plan throughout the continuum of care
- Communicate health-related information to ensure quality coordinated care and services are provided to all members
- Advocate for patients and families to ensure their needs and choices are fully represented and supported by the health care team
⚡ Requirements
- Current unrestricted RN license in the state of Texas with 2+ years of experience working within the community health setting or in a health care role
- Familiarity with Microsoft Office, including Word, Excel, and Outlook
- Ability to travel in the assigned region to visit Medicaid members in their homes and other settings
- Experience with long term care services and support, Medicaid, or Medicare is preferred
- Knowledge of federal and State requirements like the federal home and community-based settings regulations
