β¨ About The Role
- The RN Case Manager will be the primary care manager for a panel of members with complex medical/behavioral needs
- Care coordination activities will focus on supporting memberβs medical, behavioral, and socioeconomic needs
- Develop and implement person-centered care plans to address needs including management of chronic health conditions and health promotion
- Partner and collaborate with internal care team, providers, and community resources to implement care plans
- Provide education and coaching to support member self-management of care needs and lifestyle changes
β‘ Requirements
- Current, unrestricted independent licensure as a Registered Nurse in Arizona is required
- 4+ years of Clinical experience is necessary for this role
- Experience with MS Office, including Word, Excel, and Outlook is a must
- Ability to engage members telephonically and complete comprehensive needs assessments
- Strong communication skills to collaborate with internal care teams, providers, and community resources
- Experience in community case management and working with Medicaid and/or Medicare is preferred
- Background in Managed Care and DSNP experience is a plus
- Ability to remain stationary for long periods of time to complete computer or tablet work duties
