- Assess, plan, and implement care strategies individualized by patient and directed toward the most appropriate level of care
- Identify and initiate referrals for social service programs
- Manage the care plan throughout the continuum of care as a single point of contact
- Communicate health-related information to ensure quality coordinated care and services are provided
- Advocate for patients and families to ensure their needs and choices are fully represented and supported
âš¡ Requirements
- Current, unrestricted LPN/LVN license in the state of Texas
- 2+ years of clinical experience in a health care role
- Intermediate level of proficiency with Microsoft Office including Word, Excel, and Outlook
- High-speed internet access at residence
- Experience with electronic charting and arranging community resources
