⚡ Requirements

- Experienced Registered Nurse with a strong background in community health settings and case management
- Proficient in Microsoft Word and Outlook with the ability to travel up to 25% within a 60-mile radius for field visits
- Skilled in managing populations with complex medical or behavioral needs and providing community-based care management
- Certified Case Manager (CCM) certification preferred
- Familiar with NCQA standards and experienced in case management, discharge planning, and working with home care based services
