- The Long-Term Services and Supports (LTSS) Care Coordinator RN will develop person-centered, goal-oriented care strategies. - Responsibilities include assessing, planning, and implementing individualized care strategies for clients. - The role involves managing service/support plans throughout the continuum of care and ensuring timely service delivery. - The Care Coordinator will communicate with stakeholders to ensure quality coordinated care is provided. - Conducting home visits and in-person visits to various care settings is a key aspect of the job. - The position requires identifying and initiating referrals for social service programs to support clients' needs.

⚡ Requirements

- The ideal candidate will have a current, unrestricted independent licensure as a Registered Nurse in Kansas. - A minimum of 2 years of experience in a community health setting is essential for success in this role. - Experience working with individuals who have long-term care needs is required, with at least 1 year of direct involvement. - Proficiency in MS Word, Excel, and Outlook is necessary to manage documentation and communication effectively. - The candidate should possess strong advocacy skills to represent the needs and choices of individuals and families. - Ability to travel within the assigned region to conduct home visits and coordinate care is crucial.
