The Health Services and Supports Transitions Team Manager is responsible for leading and supporting several Care Coordination teams, focusing on outreach and enrollment of potential plan members and transitioning plan members from Nursing Facility to Community, Home and Community Based Settings to Community Living Support Homes, and Employment and Community First members transitioning into Community Living Support Housing. This will include execution of clinical decision processes, team management, and process improvement to meet goals of contract adherence, including submission of documentation to state partner. Responsibility extends to organizing and facilitating virtual and in-person meetings, the development and distribution of status reports, regular engagement with matrixed partners and modeling collaborative behavior and actions. This role requires a growth mindset, resilience and resourcefulness, and managing team structures, operations and processes. The Health Services and Supports Manager will report to the Long Term Services and Supports Operations Director.

This is a telecommuter position with some field responsibilities across the state of Tennessee.

Primary Responsibilities:

  • Ownership of team processes and goals, driving timelines, expectations and plan implementations
  • Drive optimizations of clinical and administrative processes to ensure quality and assure results, evaluating outcomes against benchmarks, adjusting processes and mitigating risks
  • Develop clear goals and objectives for performance management, effectively communicate expectations and hold the team accountable for results
  • Support team structures for optimal clinical and administrative coverage
  • Identify, select, structure, and prioritize process improvement projects, implementing changes to meet program requirements
  • Ensure standardized execution of workflow processes, including conducting performance audits, quality review and compliance adherence
  • Act as managerial interface with other departments to coordinate workflow processes and implementation plans

Required Qualifications:

  • Licensed staff must hold an unrestricted Registered Nurse or Social Worker license that allows them to work in Tennessee, or a 4-year Clinical Degree
  • 5+ years of clinical experience
  • 1+ years of leadership experience with responsibility for team performance
  • Intermediate MS Office skills
  • Reliable transportation and the ability to travel up to 25% within the state of Tennessee to meet with team members and leadership, members, and providers

Preferred Qualifications:

  • Master’s Degree or Higher in clinical field
  • Experience working in team-based care
  • Background in Managed Care
