✨ About The Role
- The Health Services and Supports Transitions Team Manager will lead and support multiple Care Coordination teams.
- Responsibilities include outreach and enrollment of potential plan members and transitioning members from nursing facilities to community settings.
- The role involves executing clinical decision processes and managing team structures and operations.
- The manager will be responsible for process improvement to meet contract adherence goals and submission of documentation to state partners.
- Regular engagement with matrixed partners and facilitating meetings, both virtual and in-person, is a key part of the job.
⚡ Requirements
- The ideal candidate will have a strong clinical background, with at least 5 years of clinical experience and 1 year of leadership experience.
- A valid and unrestricted Registered Nurse or Social Worker license in Tennessee is required for this position.
- The candidate should possess intermediate skills in MS Office to manage documentation and reporting effectively.
- A growth mindset, resilience, and resourcefulness are essential traits for success in this role.
- Experience in team-based care and managed care environments will be beneficial for this position.
