- The ideal candidate will have a current, unrestricted, independent licensure as a Registered Nurse.
- A minimum of 2 years of relevant clinical work experience is required, along with at least 1 year of community case management experience.
- Candidates should demonstrate knowledge of home health, hospice, public health, or assisted living.
- Proficiency in MS Word, Excel, and Outlook is essential for this role.
- The successful applicant will be willing and able to travel within the assigned region to visit Medicaid members in their homes and other settings.
