✨ About The Role
- Responsible for rounding and managing the clinical care of the population at the assigned skilled nursing facility
- Provide high-quality, patient-centered, comprehensive primary care services to patients residing in skilled nursing facilities
- Timely documentation of all patient care/encounters using Theoria's proprietary electronic health record system
- Expected to be on-site and on time for in-person scheduled shifts
- Availability for phone/secure messaging consultations with facility nursing staff
