LPN Home Manager

The LPN Home Manager is responsible for delivering compassionate nursing care and supervising day-to-day operations within IDD waiver service group homes. This role combines clinical expertise and team leadership to ensure the health, safety, and well-being of individuals with intellectual and developmental disabilities. The LPN Home Manager works closely with clinical staff, manages direct support employees, and promotes an environment that prioritizes person-centered care. This position requires a current LPN license and knowledge of IDD services, compliance standards, and the unique needs of the individuals served.

Pay: $29.00

Duties and Responsibilities:

  • Provide comprehensive nursing care, observing and reporting health conditions of individuals to the registered nurse.
  • Coordinate routine and preventative medical care, ensuring timely appointments and follow-up.
  • Document nursing care, health observations, and treatment administration in accordance with regulatory requirements.
  • Conduct quality assurance checks, including medication pass observations, and assess medication closets to maintain compliance.
  • Assist physicians during on-site clinics and facilitate communication with physicians, pharmacies, and other healthcare providers.
  • Deliver health-related training to unlicensed personnel per state regulations, ensuring competency in assigned health tasks.
  • Act as an advocate for individuals, working to develop community supports to optimize health and quality of life.
  • Supervise and provide feedback to Direct Support employees across all shifts, ensuring adherence to person-centered care standards.
  • Conduct routine monitoring and support within the residential facility, fostering a positive environment and serving as a role model for staff.
  • Provide performance reviews, resolve employee issues, and oversee staff training to ensure compliance with regulatory requirements.
  • Coordinate and participate in after-hours and on-call coverage as part of the care team rotation, ensuring continuous quality support for residents.
  • Monitor and ensure sufficient LPN staffing levels.
  • Perform routine audits of medication administration and documentation to ensure accurate and compliant record-keeping.
  • Work with healthcare and administrative staff to establish standardized care procedures, facilitating communication and feedback.
  • Ensure group home compliance with all regulatory standards, including state, federal, and accreditation requirements.
  • Coordinate and implement person-centered plans, monitoring outcomes and making adjustments to improve resident well-being.
  • Monitor select budget items to remain within program guidelines, assisting with resource allocation and cost management.
  • Maintain organized documentation of all medical and program-related activities, coordinating with interdisciplinary teams as needed.
  • Perform additional duties as assigned and enhance service quality.

Supervisory Responsibilities:

This position oversees and coordinates activities of Direct Support Professionals and Direct Support LPNs within assigned group home(s).

Minimum Qualifications:

  • Graduation from an accredited licensed nurse program.
  • Current LPN license with no restrictions and valid CPR certification.
  • Must be at least 18 years of age.
  • Minimum of one year of experience working with individuals with intellectual and developmental disabilities.
  • Excellent communication skills, with the ability to engage effectively with supported individuals, families, and interdisciplinary team members.
  • Ability to work independently, prioritize tasks, and maintain organization in a dynamic care environment.
  • Flexibility to provide on-call coverage as needed, with availability across various shifts.

Physical Demands and Work Environment:

  • Regularly required to lift 30 lbs.
  • Must be able to lift a minimum of 40 lbs.
  • Must be able to pull a minimum of 25 lbs.
  • Must be able to squat, kneel, crawl, crouch, climb, and stoop.
  • Must be able to regularly use hands to finger, handle, or feel objects, tools, or controls.
  • Required to regularly stand and walk.
  • Must be able to demonstrate proficiency in CPR from the floor level requiring to work on hands, knees, bending, standing, and lifting.
  • Vision requirements include close vision, distance vision, and peripheral vision.
  • Must be able to talk and hear.
