- The RN Case Manager will be responsible for Complex Case Management, Disease Management, and Transitional Case Management.
- This role involves conducting transition discharge assessments both onsite and telephonically to identify member needs at the time of transition to a lower level of care.
- The RN will serve as a clinical liaison with hospital, clinical, and administrative staff for discharge planning and next site of care needs.
- Engaging with members, families, and caregivers telephonically is crucial to ensure a well-coordinated action plan is established.
- The position requires collaboration with the Interdisciplinary Care Team to create individualized transition plans and action plans for members.
β‘ Requirements
- The ideal candidate will be a Registered Nurse licensed to practice in New York State with current registration and in good standing.
- A minimum of 3 years of diverse clinical experience is required, preferably in caring for acutely ill members with multiple disease conditions.
- Proficiency in working with electronic medical records is essential for this role.
- Candidates should possess knowledge of utilization management, quality improvement, and discharge planning.
- Strong skills in Microsoft Office applications, including Outlook, Word, and Excel, are necessary for effective communication and documentation.
