✨ About The Role

The Skilled Inpatient Care Coordinator (SICC) will be responsible for optimizing patients' recovery journeys by completing functional assessments, engaging with care teams, and coordinating discharge planning The role involves traveling to skilled nursing facilities in Sandy Springs, GA, to assess patients, review discharge plans, and ensure smooth transitions of care The SICC will collaborate with healthcare teams, engage with patients and families, and participate in care conferences to support patients' progress towards discharge goals Additionally, the position requires adherence to organizational policies, maintenance of accurate documentation, and staying current on regulatory policies related to care coordination The SICC will also be involved in managing caseloads efficiently, identifying opportunities for improvement, and promoting a positive work environment.

⚡ Requirements
