The Skilled Inpatient Care Coordinator (SICC) will be responsible for optimizing patients' recovery journeys by completing functional assessments, engaging with care teams, and coordinating discharge planning
The role involves traveling to skilled nursing facilities in Sandy Springs, GA, to assess patients, review discharge plans, and ensure smooth transitions of care
The SICC will collaborate with healthcare teams, engage with patients and families, and participate in care conferences to support patients' progress towards discharge goals
Additionally, the position requires adherence to organizational policies, maintenance of accurate documentation, and staying current on regulatory policies related to care coordination
The SICC will also be involved in managing caseloads efficiently, identifying opportunities for improvement, and promoting a positive work environment.
β‘ Requirements
Proven clinical experience as a Registered Nurse, Physical Therapist, Occupational Therapist, or Speech Therapist, with a minimum of 5 years in the field
Experience in case management and working with the geriatric population is highly preferred
Strong communication skills and the ability to collaborate effectively with various healthcare professionals and patients
Detail-oriented individual with exceptional problem-solving and decision-making skills
Ability to work independently, prioritize tasks, and manage multiple responsibilities simultaneously
