β¨ About The Role
- The Skilled Inpatient Care Coordinator (SICC) plays a crucial role in optimizing patients' recovery journeys by completing functional assessments and engaging with the post-acute care team
- The SICC collaborates effectively with the patients' healthcare teams to establish optimal discharge plans and ensure efficient transitions of care
- This role involves reviewing targets for Length of Stay (LOS), target outcomes, and discharge plans with providers and families
- The SICC engages with patients, families, or caregivers weekly to assess progress towards discharge goals and assist in resolving barriers
- The position requires adherence to organizational and departmental policies and procedures, as well as maintaining confidentiality of all PHI information
β‘ Requirements
- Active, unrestricted registered clinical license required in the state of hire, such as Registered Nurse, Physical Therapist, Occupational Therapist, or Speech Language Pathologist
- 5+ years of clinical experience is necessary for this role
- Residing within or near the county listed on the job description is preferred
- Experience working with the geriatric population and a background in patient education, rehabilitation, and/or home health nursing is beneficial
- Proficiency with Microsoft Office applications including Outlook, Excel, and PowerPoint is required
