✨ About The Role

- The Field Case Manager RN will be responsible for conducting initial and ongoing assessments in a home setting, identifying health risks, and developing personalized care plans for patients - The role involves collaborating with primary providers and multidisciplinary teams to align goals and drive consistent coordination of care - The Field Case Manager RN will assess consumer data, identify gaps in care, and prioritize interventions and resources to empower consumers to meet their health goals - The position requires maintaining consumer engagement, establishing rapport, demonstrating empathy, and building trusting relationships with patients - The Field Case Manager RN will also provide ongoing support for advanced care planning and reassess care plans at appropriate intervals based on patient progress

⚡ Requirements

- Experienced RN with at least 5-8 years in the healthcare industry, particularly working with complex patient populations and chronic diseases - Strong ability to work independently and in a home-visit setting, effectively communicating with elderly and chronically ill patients and families - Comfortable navigating a Windows-based environment and documenting patient care in appropriate EHR systems - Excellent organizational skills, able to multitask, manage schedules, and collaborate effectively with care teams and physicians - Holds an active, unencumbered RN license to practice nursing in South Carolina and has access to reliable transportation
