- The Cross-Market Care Coordinator plays a dual role as a Skilled Inpatient Care Coordinator (SICC) and Transitional Care Coordinator (TCC)
- Responsible for assessing, planning, implementing, coordinating, monitoring, and evaluating options and services to meet patients' post-acute health needs
- Collaborates with in-market leaders, hospital care teams, physicians, and post-acute care providers to ensure safe transitions of care and improve patient outcomes
- Utilizes proprietary technology and evidence-based tools to determine appropriate levels of care and interventions
- Provides telephonic post-discharge support, conducts functional assessments, and develops individualized transitional care plans for patients

- Experienced healthcare professional with a background in clinical care coordination and transitional care planning
- Strong communication skills to effectively engage with patients, families, and interdisciplinary healthcare teams
- Ability to work independently and collaboratively in a fast-paced environment
- Detail-oriented with a focus on providing quality, cost-effective outcomes for patients
- Knowledge of federal and state regulatory policies related to utilization management and care coordination
