- Provide leadership in coordinating patient-centered care across the continuum and developing safe discharge plans
- Screen patients for admission, concurrent medical necessity, and post-acute care needs
- Maintain knowledge of current care management practices, care coordination changes, and regulatory requirements
- Assess physical, psychosocial, and economic needs for transition of care planning to various levels of care
- Serve as an educational resource to all Health System staff regarding utilization review and discharge planning
β‘ Requirements
- Experience in nursing with at least 3-4 years of relevant experience in patient care coordination and discharge planning
- Ability to work autonomously and as a high performer in a multi-disciplinary team environment
- Strong problem-solving skills to identify solutions to non-standard requests and problems
- Current RN license in the State of Maine or compact state licenses with BLS certification
- Demonstrated ability to provide patient advocacy and develop optimal transition plans for patients to reduce readmissions
