This position requires networking and collaborative opportunities for analysis and assessment of community needs. The goal is to help build a healthier community through health promotion and disease prevention efforts.

The Nurse will:

  • Participate in the creation, implementation and improvement of health education and screenings in community settings.
  • Assesses the needs of patients to determine appropriate intervention; provide clinical triage, manage patient referrals at health screening events, provide vaccines and other services that promote optimal allocation of health care services through effective and efficient use of resources; Assist with clinical follow-up.
  • In collaboration with the Clinical Nurse Manager, recruit, orient, and oversee competencies of nurses, volunteers and students for the events.
  • Collaborate and communicate with department leadership to develop and maintain strong working relationships with community and department leaders.
  • Attend meetings, make presentations, develop reports and maintain required statistics and records for the department.
  • Collect data, track and evaluate the effectiveness of programs.

This is a full-time position that includes evening and weekend programs.

Preferred Criteria:

  • Master's degree in Nursing or related field
  • Ambulatory care and experience in primary or community health
  • Ambulatory Care Board Certification
  • Bilingual – English/Spanish

Required Criteria:

  • Bachelor's degree in Nursing
  • Current New York State RN license
  • Current BLS through the AHA
  • 3 plus years of clinical experience
  • Excellent oral and written communication skills
  • Proficient in Microsoft Excel, Word and PowerPoint (reports/spreadsheets)
  • Leadership and program coordination experience

Please note that all roles require on-site presence (variable by role). Therefore, all employees should live within a commutable distance to NYP. NYP will not reimburse for travel expenses.
