✨ About The Role
- The Home Health Registered Nurse provides direct patient care in the patient's home under physician supervision
- Responsibilities include following start of care plans, providing treatment, discharges, wound care, medication administration, and collaborating with a clinical team
- The role involves case management, developing treatment plans, submitting progress reports, and discharging patients after consultation with the physician
- The nurse will participate in and monitor patient and family care compliance activities and professionally manage and coordinate the plan of care
- Completing clinical and medical documentation, identifying and utilizing resources and referrals, and ensuring positive patient care outcomes are essential duties
âš¡ Requirements
- Minimum of 1 year RN experience in an institutional setting, such as Acute Care, Rehab, Hospital, Home Health, or Hospice
- Ability to manage and coordinate the plan of care, complete clinical and medical documentation, and identify and utilize resources and referrals
- Experience in wound care, patient teaching, medication management, lab draws, and IV infusion is a plus
- Proficiency in charting with Electronic Medical Record Software, such as Wellsky or Kinnser, is advantageous
- Current and unrestricted California Registered Nurse license and CPR/BLS certification are required
