✨ About The Role
- Provide direct patient care in the patient's home under physician supervision
- Develop treatment plans based on physician's orders and initial patient assessment
- Submit regular patient care progress reports to attending physician and clinical staff
- Discharge patients after consultation with the physician and clinical manager
- Participate in and monitor patient and family care compliance activities
âš¡ Requirements
- Minimum of 1 year RN experience in an institutional setting, preferably in Acute Care, Rehab, Hospital, Home Health, or Hospice
- Ability to manage and coordinate patient care plans effectively
- Experience in wound care, patient teaching, medication management, lab draws, and IV infusion is a plus
- Proficiency in charting with Electronic Medical Record Software, such as Wellsky or Kinnser, is advantageous
- Strong communication skills and the ability to work collaboratively with a clinical team
