Registered Nurse (Home Health) - Santa Monica, CA

✨ About The Role

- The Home Health Registered Nurse will provide direct patient care in the patient's home under physician supervision - Responsibilities include following start of care plans, providing treatment, discharges, wound care, medication administration, and collaborating with a clinical team for positive patient care outcomes - The role involves case management, developing treatment plans, and submitting regular patient care progress reports - The Nurse will discharge patients after consultation with the physician and clinical manager, provide nursing care and written care plans, and monitor patient and family care compliance activities - Additionally, the Nurse will complete clinical and medical documentation on all patients and appropriately identify and utilize resources and referrals

⚡ Requirements

- Minimum of 3-4 years of experience in a relevant industry job, such as Acute Care, Rehab, Hospital, Home Health, or Hospice - Ability to manage and coordinate the plan of care effectively - Experience in wound care, patient teaching, medication management, lab draws, and IV infusion is a plus - Proficiency in charting with Electronic Medical Record Software, such as Wellsky or Kinnser, is advantageous - Strong communication skills and ability to work collaboratively with a clinical team
