Registered Nurse (Home Health) - Lancaster, CA

✨ About The Role

- The Home Health Registered Nurse will provide direct patient care in the patient's home under physician supervision - Responsibilities include following start of care plans, providing treatment, discharges, wound care, medication administration, and collaborating with a clinical team for positive patient care outcomes - The role involves case management, developing treatment plans, submitting progress reports, discharging patients, and providing nursing care and written care plans - The nurse will participate in and monitor patient and family care compliance activities, professionally manage and coordinate the plan of care, and complete clinical and medical documentation - The position requires a current and unrestricted California Registered Nurse license, CPR/BLS certification, and a minimum of an Associate degree in Nursing

⚡ Requirements

- Minimum of 1 year RN experience in an institutional setting, such as Acute Care, Rehab, Hospital, Home Health, or Hospice - Ability to manage and coordinate the plan of care, complete clinical and medical documentation, and identify and utilize resources and referrals - Experience in wound care, patient teaching, medication management, lab draws, and IV infusion is a plus - Proficiency in charting with Electronic Medical Record Software, such as Wellsky or Kinnser, is advantageous - Strong compliance history and ethical values are essential for success in this role
