✨ About The Role
- The Home Health Registered Nurse will provide direct patient care in the patient's home under the supervision of a physician
- Responsibilities include following start of care plans, providing treatment, discharges, wound care, medication administration, and working collaboratively with a clinical team to achieve positive patient care outcomes
- The role involves case management, follow-up skilled nursing visits, and developing treatment plans based on physician's orders and initial patient assessment
- The Registered Nurse will be responsible for submitting regular patient care progress reports to attending physician and clinical staff, as well as discharging patients after consultation with the physician and clinical manager
- The position requires completing clinical and medical documentation on all patients and professionally managing and coordinating the plan of care
âš¡ Requirements
- Minimum of 1 year RN experience in an institutional setting, such as Acute Care, Rehab, Hospital, Home Health, or Hospice, is required
- Experience in wound care, patient teaching, medication management, lab draws, and IV infusion is a plus
- Ability to chart with Electronic Medical Record Software (Wellsky / Kinnser a plus) is beneficial
- Strong background in medical specialties like Critical & Intensive Care, Emergency Medicine, Geriatrics, Home Health, and Medical-Surgical
- Ideal candidate should have experience in a physical setting such as Acute care, Hospital, Long term care, Outpatient, or Urgent care center
