- The role involves working with members, providers, and multidisciplinary team members to assess, plan, and coordinate an integrated delivery of care
- Responsibilities include completing comprehensive assessments of members, developing case management plans, and conducting ongoing monitoring of care plans
- The job requires promoting integration of services for members, facilitating interdisciplinary care team meetings, and conducting motivational interviewing
- RNs in this role are assigned cases with members who have complex medical conditions and medication regimens
- The position also involves conducting medication reconciliation when needed
                            

⚡ Requirements

- Ideal candidate will have 3-4 years of experience in case management, disease management, managed care, or medical/behavioral health settings
- Must possess an active, unrestricted State Registered Nursing (RN) license in good standing
- Strong ability to conduct comprehensive assessments of members and develop case management plans in collaboration with healthcare professionals
- Experience in conducting face-to-face or home visits as required
- Skilled in ongoing monitoring of care plans, evaluating effectiveness, and suggesting changes accordingly
