✨ About The Role
- The role involves working with members, providers, and multidisciplinary team members to assess, plan, and coordinate an integrated delivery of care
- Responsibilities include completing face-to-face comprehensive assessments, facilitating waiver enrollment processes, and developing case management plans
- Monitoring care plans, evaluating effectiveness, and suggesting changes accordingly are key aspects of the job
- The position requires assessing for medical necessity, authorizing appropriate waiver services, and conducting home visits as required
- Collaborating with interdisciplinary care teams, providing consultation, and educating non-RN case managers are also part of the job duties

- Experience working with individuals with disabilities/chronic conditions and Long Term Services & Supports is required
- Strong background in case management, disease management, managed care, or medical/behavioral health settings
- Ability to conduct face-to-face comprehensive assessments and develop case management plans
- Skilled in promoting integration of services for members including behavioral health care and long term services and supports
- Comfortable with 50-75% travel requirement for the role
