- The Case Manager RN will work in a remote setting, primarily supporting the Medicaid High Risk OB population.
- Responsibilities include conducting comprehensive assessments of members and developing case management plans in collaboration with healthcare professionals.
- The role requires ongoing monitoring of care plans to evaluate effectiveness and suggest necessary changes.
- Local travel to the office in Troy, MI may be required for team meetings, audits, and training sessions.
- The position operates on a Monday through Friday schedule from 8:30 AM to 5:00 PM EST, with no weekend or holiday work expected.
⚡ Requirements
- The ideal candidate will be a Registered Nurse (RN) with a current active unrestricted RN license in the state of Michigan.
- Experience in case management, disease management, or managed care settings is essential, ideally with 1-3 years of relevant experience.
- Strong computer skills and attention to detail are crucial for multitasking between systems and accurately documenting member interactions.
- The candidate should possess excellent communication skills to effectively engage with members over the phone and during home visits.
- A background in supporting patients with high-risk pregnancies, obstetrics (OB), and neonatal intensive care unit (NICU) is preferred.
