- The Care Connections Nurse Practitioner will focus on delivering screening and preventive primary care services in non-clinical settings such as homes and nursing facilities.
- Responsibilities include performing comprehensive medical assessments, formulating treatment plans, and coordinating care for various patient populations.
- The role requires the ability to address both chronic and acute primary care complaints and establish reasonable medical diagnoses.
- The Nurse Practitioner will be responsible for ordering diagnostic tests and performing pertinent laboratory and radiology testing.
- The position involves driving up to 120 miles daily to visit patients in various locations, with potential overnight stays for special projects.
- Participation in community-based "Pop Up Clinics" is expected to build relationships and address healthcare gaps in the community.

- The ideal candidate will have a Master’s degree in family health from an accredited nursing program.
- A minimum of 3-5 years of experience as a Registered Nurse and/or Nurse Practitioner is preferred, particularly in home health or community health settings.
- Candidates should possess advanced computer skills, including proficiency with Word, Excel, and Electronic Medical Records.
- An active and unrestricted national certification as a Nurse Practitioner is required, along with a current state-issued license.
- Experience working with underserved populations and managing chronic conditions will be beneficial.
- Fluency in a language other than English is a plus, enhancing communication with diverse patient populations.
