Coordinates palliative care consults for hospital patients with both chronic illness and serious illness/injury. Initiates advance care planning with patients to promote discussion of patients wishes related to treatment options with patients, family, physicians, and other health care providers. Documents patients wishes in chart and provides patient and family with ACP tool for further discussion and reference. Coordinates with discharge planners, homecare, and hospice representative to assure patients care is provided in the most appropriate setting.
Education: Graduate of an accredited school of nursing, bachelors degree preferred
Licensure: Current professional license in Kentucky
BLS certification required
