Summary:

Under minimal supervision and according to established nursing procedures, provides health education to health screening participants in a variety of settings. Health screenings performed may include blood pressure, blood sugar, total cholesterol, HDL, total cholesterol/HDL ratio, BMI, bone density and others. Facilitates six-week workshops on self-management of chronic disease and diabetes using an evidence-based curriculum. Provides care to pediatric, adult and geriatric patients.

Education:

Required: Associates degree in Nursing, Bachelors in Nursing preferred.

License: State of Ohio Registered Nurse license required. BLS certification required.

Experience:

Minimum experience preferred: 2 years of nursing

Broad knowledge base about factors that influence the level of personal wellness in general along with cardiac disease, diabetes, and obesity in particular. Ability to make presentations. Ability to travel.

Hours:

Various work hours including days, evenings and weekends in community settings.
