Responsible for the coordination of multidisciplinary specialty programs and care continuum for patients diagnosed and/or receiving treatment in the specialty area. Coordinates and manages patient care in collaboration with a multidisciplinary team of healthcare providers. 3 years RN experience Transplant experience
Ensures timely scheduling of diagnostic work-up, results reporting, and communications between patients and the multidisciplinary team. Navigates the care continuum for patients by collaborating with the multidisciplinary care team. Coordinates assessment and evaluation of patients for proper care services including, but not limited to, tests, procedures, diagnostic labs. Coordinates and ensures thorough and consistent follow-up care of patients with the appropriate care providers. Provides education and counseling to patients and their families about the disease, treatment side effects, and coping strategies. Assesses potential learning problems (language barrier, readiness to learn, etc.) and structures teaching sessions to meet individual needs. Assesses patients' physical and emotional status, symptoms related to the disease and its treatment, and patients' understanding of the disease and treatment plan. Provides education about disease-specific services to the patients and families along with the community at large.

Accredited Program: Nursing (Required) Registered Nurse Compact License (RN LICENSE COMPACT) - Compact RN Multistate, Registered Nurse License (RN LICENSE) - State of Florida (FL)
Complexity of Work: Requires critical thinking skills, effective communication skills, decisive judgment, and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action. Required Work Experience: One (1) year of nursing experience in the following area(s): critical care, oncology, or any nursing specialization in the abdominal/thoracic organs.
