RN Case Manager

The RN Case Manager coordinates the care and service of patient populations from admission through discharge. The RN Case Manager, collaboratively with inter-disciplinary teams, works to build a comprehensive case management plan through effective care coordination and utilization of healthcare resources to achieve desired clinical and financial goals. Key responsibilities are to partner with the healthcare team to ensure all aspects of the patient's needs, clinical, psychosocial, and financial are adequately addressed in the transition of the care plan and to manage the patient's timely progression of care and safe transition to the next most appropriate level of care.

Responsibilities include:

• Collaborating with the Social Worker to proactively identify the need for team and/or patient and family conferences to facilitate discussion of the patient's condition, discuss prognosis and determine an agreed upon transition of care plan.
• Clinically assessing, planning, implementing, coordinating, monitoring, and evaluating options and services to meet an individual's health needs to facilitate a safe and timely discharge to the next appropriate post acute level of care.
• Conducting an in-depth case management assessment of a patient's needs at the time of admission and throughout the patient's stay.
• Addressing system-level issues impeding diagnostic or treatment progress with the healthcare team and reporting unresolved opportunities for improvement through the organizational defined escalation process.
• Proactively identifying and resolving barriers to timely discharge/transition and documenting avoidable delays information in accordance with health system protocols.
• Performing duties in a manner that promotes quality patient care/satisfaction, while promoting safety, cost efficiency, and a commitment to the continuous quality improvement process.
• Monitoring patient and family satisfaction. Responding to questions and complaints from patients, family members, and payers regarding care.
• Actively coordinating progress and the patient's care by monitoring the length of stay (LOS) of the patient's hospitalization, leads and facilitates rounds, and proactively works to meet expected length of stay and clinical targets/indicators.
• Performing ongoing chart review to identify actual or potential issues, which may include service delivery, patient outcomes, satisfaction, compliance, cost, and reimbursement.
• Consulting with Physicians and multidisciplinary teams regularly to evaluate the patient’s status and appropriateness of medical care, including admission, length of stay (LOS), transfer, and discharge.

• Required Work Experience: Minimum one (1) year of experience in a hospital-based Case Management practice or five (5) years in Healthcare.
• Other Information: Additional Education Info: Graduate of an accredited Registered/Professional Nursing Program. Additional Credential Info: Basic Life Support (BLS) Healthcare Provider required upon hire.
