Summary: Acts as a resource to all levels of staff and management in evaluating patient care and nursing practice standards. Provides clinical and administrative supervision to the case managers assigned on unit including ongoing performance evaluations of staff. Is involved in program planning, feedback on both policymaking, and decision-making for the unit and/or department. Communicates information to unit staff as well as to the manager.
Responsibilities: Assists with supporting the department manager and director through the evaluation of department operations, including information technologies, service level determination, and complaint management, to achieve performance and quality control objectives. Analyzes and submits statistical reports monthly and as requested. Collaborates with interdisciplinary staff to design and implement appropriate treatment plans, including discharge planning, to ensure quality outcomes. Develops, leads or co-leads groups as needed. Supports and maintains existing standards of the department and of the profession. Participates in the orientation, training, evaluation, clinical performance improvement, disciplinary actions, and continuing education initiatives of the case management staff. Manages and constantly evaluates staffing levels, assignments, skills, learning needs, and deployment of staff. Performs problem resolution activities to maintain quality patient care. Identifies practice and system issues, and trends utilizing medical criteria, protocols, and clinical data systems utilized by the organization. Assists with the development and implementation of Case Management goals, plans, and standards consistent with the clinical, administrative, regulatory, legal, and ethical requirements.

Additional Job Information: Complexity of Work: Plays a pivotal role in overseeing the daily operations of the case management team ensuring that all activities align with organizational objectives and meet regulatory standards. Requires critical thinking skills, effective communication (verbal and written) skills, decisive judgment, and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action. Have knowledge of State and Federal regulations governing discharge planning conditions of participation, hospital information systems (e.g., logician and IDX), staff development techniques, and performance improvements processes. Requires a comprehensive understanding of medical necessity criteria, payer guidelines, and regulatory compliance standards. This role involves high level analytical and decision making skills to evaluate clinical data against medical necessity criteria while balancing financial and operational objectives. Responsibilities include real time problem solving and prioritizing urgent cases, coordinating with physician advisors on challenging cases, or escalating to leadership as needed. Required Work Experience: Three (3) years of experience in an acute care setting as an RN in case management. Leadership experience preferred. Other Information: Additional Education Info: Graduate of an accredited Registered/Professional Nursing program or a Bachelor's degree in Nursing (BSN). Additional Certification Info: Certification in Case Management preferred.
