✨ About The Role
- The Registered Nurse will provide clinical support to Community Health Navigators (CHNs) in managing high-risk members through the Enhanced Care Management (ECM) program.
- Responsibilities include conducting clinical rounds, leading training sessions, and offering guidance on complex care decisions.
- The role focuses on empowering CHNs to deliver effective care and achieve positive member outcomes without direct patient care involvement.
- The position may require travel up to 10% for operations meetings or field support.
- The RN will assist in onboarding new CHNs and provide ongoing education to ensure they are equipped to manage members with chronic health conditions.

- The ideal candidate will have an active Registered Nurse (RN) license in California or the ability to obtain it within 10 weeks of employment.
- A Bachelor of Science in Nursing (BSN) is preferred, with a Master of Science in Nursing (MSN) being a plus.
- Candidates should have a minimum of 2 years of experience in a clinical or community health setting, particularly in managing chronic health conditions.
- Experience in training and education is highly desired, indicating a strong ability to lead and support Community Health Navigators (CHNs).
- Familiarity with CALAIM, ECM, and HEDIS measures will be beneficial for success in this role.
