- The Special Programs Case Manager I will perform case management telephonically and through home visits within the scope of licensure for special programs like Foster Care.
- The role involves conducting assessments to identify individual needs and developing comprehensive care plans.
- The case manager will coordinate with internal and external resources to ensure members have access to quality and cost-effective care.
- The position requires some travel within the assigned region for facility or home visits.

- A successful candidate will have a Master's degree in social work, counseling, or a related behavioral health field, or a degree in nursing.
- The role requires a minimum of three years of clinical experience in social work counseling, particularly with complex psychiatric and substance abuse disorders.
- Candidates must possess an active and valid license issued by the Commonwealth of Virginia, such as LCSW, LMSW, LPC, or RN.
- Experience working with specialty populations and knowledge of the Virginia Foster Care system will be beneficial.
- Strong communication skills are essential for building partnerships with designated populations and their families.
