✨ About The Role
- The RN Nurse Case Manager II will work Monday to Friday from 10:30 am to 7:00 pm Central Time, primarily in a telephonic capacity.
- Responsibilities include assessing, developing, implementing, coordinating, monitoring, and evaluating care plans for members with complex and chronic care needs.
- The role involves conducting assessments to identify individual needs and creating specific care management plans.
- The Nurse Case Manager will coordinate internal and external resources to meet identified needs and monitor the effectiveness of care management plans.
- The position requires interfacing with Medical Directors and Physician Advisors to develop treatment plans and assist in problem-solving with providers and claims issues.
⚡ Requirements
- The ideal candidate will have a bachelor's degree in a health-related field and at least 5 years of clinical experience.
- A current, unrestricted RN license in the state of residence is required, along with multi-state licensure or a compact license.
- Candidates with certification as a Case Manager and formal case management experience within managed care or hospital settings will be preferred.
- A strong clinical background in hospital settings such as ER, ICU, or Critical Care is essential for success in this role.
- The successful candidate will be adept at telephonic care management and possess excellent communication skills to interact with members and healthcare providers effectively.
