- The LTSS Service Coordinator - RN is responsible for managing members' cases within the scope of licensure.
- The role involves performing clinical assessments to identify and manage members' needs across various health domains.
- The RN will develop, monitor, evaluate, and revise care plans to optimize member health care.
- Coordination of care for members with chronic illnesses and disabilities is a key responsibility.
- The position requires collaboration with medical directors, physician advisors, and interdisciplinary teams to develop treatment plans.
âš¡ Requirements
- The ideal candidate will have a current, unrestricted RN license in the applicable state(s).
- A minimum of 3 years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities is required.
- Experience in a Service Coordinator, Case Management, or similar role is essential for success in this position.
- Candidates with a Master's degree in Health or Nursing will be preferred.
- Strong communication and interpersonal skills are necessary to effectively coordinate care with members and healthcare teams.
