- The LTSS Service Coordinator - RN will manage members' cases within the scope of their licensure.
- Responsibilities include performing clinical assessments to identify and coordinate member needs across various health services.
- The role involves managing care for members with chronic illnesses and ensuring efficient utilization of health benefits.
- The RN will develop individual care plans and establish goals in collaboration with members and their families.
- The position requires interfacing with medical professionals to develop care management treatment plans.

- The ideal candidate will have a current, unrestricted RN license in the applicable state(s).
- A minimum of 3-4 years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities is essential.
- Experience in a Service Coordinator, Case Management, or similar role is highly preferred.
- Strong communication skills are necessary for effective telephonic and face-to-face clinical assessments.
- The candidate should be capable of developing and revising care plans in collaboration with members and their support systems.
