✨ About The Role
- The LTSS Service Coordinator-RN Clinician will manage members' cases within the scope of their licensure.
- Responsibilities include developing, monitoring, evaluating, and revising care plans to meet members' needs.
- The role requires up to 75% travel within the assigned areas of Jamaica and the Bronx.
- The RN will identify high-risk members and coordinate care with the healthcare team.
- The position involves supervising non-RN clinicians and ensuring the quality of care provided to members.
âš¡ Requirements
- The ideal candidate will have a current, unrestricted RN license issued by the state of New York.
- A minimum of three years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities is essential.
- Experience in a Service Coordinator, Case Management, or similar role is highly desirable.
- Strong communication skills are necessary for effective interaction with members, caregivers, and healthcare teams.
- The candidate should be comfortable with both telephonic and face-to-face clinical assessments.
