✨ About The Role
- The LTSS Service Coordinator-RN Clinician will manage members' cases within the scope of their licensure.
- This role requires up to 75% travel within the assigned areas of Flushing, Queens, and the Bronx.
- Responsibilities include performing clinical assessments to identify and manage members' needs across various health domains.
- The RN will establish both short- and long-term goals in collaboration with members and their families.
- The position involves interfacing with medical directors and interdisciplinary teams to develop care management treatment plans.
âš¡ Requirements
- The ideal candidate will have a current, unrestricted RN license issued by the state of New York.
- A minimum of three years of experience working with individuals who have chronic illnesses, co-morbidities, and/or disabilities is essential.
- Strong communication skills are necessary for conducting telephonic or face-to-face clinical assessments.
- The candidate should be adept at developing, monitoring, and revising care plans in collaboration with members and their support systems.
- Experience in coordinating care with healthcare teams and managing complex cases will be beneficial.
