✨ About The Role
- The LTSS Service Coordinator will work under the supervision of an RN and will be responsible for managing member cases.
- This role involves conducting telephonic or face-to-face assessments to identify and evaluate member needs.
- The coordinator will assist in identifying high-risk members and provide clinical data to the responsible RN.
- Participation in care coordination for members with chronic illnesses and disabilities is a key responsibility.
- The position requires travel within specified counties in Florida, making a valid driver's license necessary.

- The ideal candidate will have a current, unrestricted LPN/LVN, LSW, LCSW, LMSW, or other applicable license as required by state law.
- A minimum of 2 years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities is essential for success in this role.
- Strong decision-making skills and an understanding of disease processes and clinical guidelines are important for effective case management.
- Bilingual candidates, particularly those fluent in Spanish, will be strongly preferred for this position.
- Previous experience in case management or oncology will be advantageous for applicants.
