✨ About The Role
- The LTSS Service Coordinator will work under the supervision of an RN and be responsible for managing member cases.
- This role involves conducting telephonic or face-to-face assessments to identify and evaluate member needs.
- The coordinator will assist in identifying members at high risk for complications and provide clinical data to the RN.
- Participation in care coordination for members with chronic illnesses and disabilities is a key responsibility.
- The position requires travel within specific counties in Florida based on the candidate's location.

- The ideal candidate will have a current and unrestricted LPN/LVN, LSW, LCSW, or LMSW license as required by state law.
- A minimum of 2 years of experience working with individuals who have chronic illnesses, co-morbidities, or disabilities is essential.
- Candidates should possess strong decision-making skills and an understanding of disease processes and clinical guidelines.
- Bilingual candidates, particularly those fluent in Spanish, will be strongly preferred.
- Previous experience in case management or oncology is highly desirable.
