- Responsible for assessing and addressing the physical, psychosocial, and environmental needs of home health care patients
- Develop and implement comprehensive plans of treatment for assigned caseload, including initial evaluations and follow-up visits
- Communicate effectively with clinical supervisors, physicians, and other team members regarding patient care issues and changes in health condition
- Maintain patient and staff privacy and confidentiality in accordance with HIPAA regulations
- Contribute to the success of Elara Caring by aligning activities with the organization's vision and providing unparalleled care to patients
                            

⚡ Requirements

- Experienced Registered Nurse with a minimum of 5-8 years in a clinical care setting, preferably in home health or hospice
- Strong commitment to providing high-quality care and making a positive impact on patients' lives
- Ability to work collaboratively in a team environment and lead a team of healthcare providers effectively
- Detail-oriented with excellent communication skills to effectively communicate with patients, families, and other healthcare professionals
- Must have a valid RN license, reliable transportation, and be willing to travel within the coverage area
